The strongest development: a late-stage trial that held up over two years
The most significant verified development in this batch of biotech news is late-stage clinical evidence. On October 22, 2026, Vor Bio (Nasdaq: VOR) announced final two-year results from its Phase 3 TELIGAN trial of telitacicept in IgA nephropathy, a kidney disease.1 Doctors track kidney function with a measure called eGFR, which estimates how well the kidneys filter blood. In the trial, patients on the drug lost 0.6 units of eGFR per year, against 5.7 for patients on placebo. That is a difference of 5.1 units a year, with a p-value below 0.001.1 A p-value that small means the gap is very unlikely to be down to chance.1
The company also reported 89% and 81% reductions in the risk of confirmed eGFR declines of at least 30% and at least 40%, respectively.1 It said reductions in protein in the urine were sustained through 104 weeks, with a favourable safety profile.1
One caution on trust. This is a company press release, distributed through a wire service. Across the 6,322 claims from this source type that we have checked, 53% held up.1 The numbers are specific and the trial is described as final, but we have not seen independent confirmation or the full dataset. Treat it as a promising, company-reported result.
Incyte: what a profitable biotech looks like in its filings
Incyte offers the clearest checked financial picture in the dossier, because its figures come from SEC filings. Its net income was $597.6 million in fiscal 2023, fell to $32.6 million in 2024, and reached $1.287 billion in 2025.2 That is roughly 39 times the previous year's profit. Earnings per share moved from $2.65 to $0.15 to $6.41 over the same three years.3
Cash generated by the business shows the same pattern. Operating cash flow was $496.5 million in 2023, $335.3 million in 2024 and $1.413 billion in 2025.4 Cash on the balance sheet has swung more. It stood at $3.21 billion at the end of 2023, dropped to $987.3 million at the end of the second quarter of 2024, and was $1.69 billion at the end of that year.5 By the end of 2025 it was $3.10 billion, and in the first quarter of 2026 it was $3.46 billion, roughly double the end-2024 figure.5 The filings give the figures but not the reasons for the swings, so we do not speculate about them.
One data point deserves care. Capital expenditure is recorded as exactly $48.7 million in both fiscal 2024 and fiscal 2025.6 Both are labelled as filing-sourced, but identical figures in consecutive years are unusual. We report them as recorded and would want to see the original filing before leaning on them.
Incyte's science news arrived in the same period. At the ESMO Congress 2026 it announced new results for its KRAS G12D inhibitor INCB161734 in pancreatic and colorectal cancer, and for a bispecific antibody called INCA338901 in one type of colorectal cancer, across three rapid oral sessions.7 In the pancreatic cancer data (cut-off July 6, 2026; n=54), INCB161734 was combined with standard chemotherapy in metastatic disease. Incyte described the efficacy as encouraging and the safety as favourable.8 Those are the company's words, and the dossier contains no response-rate figures to test them against. Incyte also announced collaborations with Bristol Myers Squibb and Synnovation to test INCB161734 alongside two other cancer drugs.9
The royalty earners: steady money without running the trial
Not every biotech earns money by selling its own drug. BioArctic of Stockholm receives royalties on Leqembi, the Alzheimer's drug sold by its partner Eisai. For the second quarter of 2026, Eisai recorded sales of JPY 29.3 billion. BioArctic said this resulted in a royalty of SEK 179 million, “an increase of approximately 10 percent compared to the royalty recorded by BioArctic for the second quarter 2025.”10
BioArctic points out that the comparison quarter was flattered. The second quarter of 2025 included a one-time stockpiling effect in China of about JPY 5.3 billion. Excluding it, BioArctic says, “royalty revenues in Q2 2026 grew approximately 43 percent compared to the same quarter last year.”10 Both figures come from the same release. The 10% headline and the 43% underlying growth are the company's own presentation, and the 43% depends on its adjustment for the China stockpiling.
The topic brief also names Lilly's weight-loss and diabetes demand and VivoSim's Lilly milestones as revenue anchors. Our dossier holds no verified figures on either, so we make no claims about them. Likewise, it contains nothing on index weightings, so we cannot tell you whether you already own these companies through a fund.
The early pipeline: dates are promises, not results
Several smaller companies gave timetables, which are intentions and not outcomes. Ernexa Therapeutics said it “remains on schedule” to submit its application to begin human trials (an IND) for ERNA-101 in the third quarter of 2026, with a first-in-human Phase 1 trial expected in the fourth quarter.11 Prelude Therapeutics expected to file an IND for PRT13722 by mid-2026 and to start a Phase 1 study in ER-positive breast cancer in the second half of 2026.12 Akari Therapeutics is further back. It plans Phase 1 development of AKTX-101 in mid-2027.13 Its CEO's pitch is that patients whose cancer has stopped responding to antibody-drug conjugates are “a significant, growing unmet need” and an exclusive opportunity for its novel payload, which, the company says, most current and future drugs of that type cannot access.13
It is now November 2, 2026. Ernexa's third-quarter and Prelude's mid-year deadlines have passed, and the dossier does not confirm that either filing happened. Those are the first things to check.
OPKO Health reported that its candidate OPK8801001 showed approximately 20-fold greater growth hormone receptor antagonism than the established therapy pegvisomant. The company stressed that this was “In vitro”, meaning in laboratory conditions and not in patients.14 That is a long way from a result in people.
The talent build-out, and a note on insider sales
Companies are also adding leadership. Idorsia appointed Begoña Carreño-Gómez as Chief Business Development Officer and a member of its Executive Committee, effective November 1, 2026.15 The topic brief mentions a change of CEO at Idorsia, but the dossier we hold records only this appointment, so we do not report a CEO change.
A different kind of leadership signal: UroGen Pharma's Chief Medical Officer, Mark Schoenberg, sold 10,000 ordinary shares for a total of $400,000 on July 9, 2026.16 Insider sales can alarm readers, so the context matters. The sale was made under a pre-arranged trading plan adopted on August 15, 2025, and it was the final transaction in that plan's schedule.16 A plan fixed a year earlier carries less information about current sentiment than a spontaneous sale would.
AI-designed drugs and bold rejuvenation claims: the thinnest evidence
The emerging thread is the least proven. Radical Numerics says AI-designed viruses have treated its first patient, with an experimental treatment aimed at drug-resistant E. coli.17 This is one company's statement carried in a technology newsletter, with no outcome data. The newsletter source has a poor record in our checks: only 6% of 16 checked claims held up.17 A first patient treated is a milestone for the company. It is not evidence that the treatment works.
The same caution applies to Generation Lab, whose fact sheet says its treatment “blocks the systemic spread of aging in the bloodstream, reawakens the body's own repair mechanism and restores health and youth to multiple tissues.”18 That is the company's marketing language. The MIT Technology Review article that reported it is headlined “A startup claims it's found a drug to make your blood young” and presents the line as a claim, not an established result.18 We include it because it shows how the sector's language can run ahead of its evidence.
What to watch
- Whether Ernexa and Prelude confirm the IND filings and Phase 1 starts that their own timetables placed in the third quarter and mid-2026.
- Incyte's analyst and investor event on October 26, 2026, which was scheduled to present results and additional updates.19
- Whether Leqembi royalties keep growing once the China stockpiling comparison is behind them.
- Independent confirmation of the TELIGAN data beyond the company release.
- Any outcome data, not just a first-patient announcement, from AI-designed therapies.
